Obstetric Department carries out analgesic labor in various ways to reduce labor pain and promote the concept of natural labor, significantly reducing cesarean section rate to 33.78%; the natural delivery rate is 66.22%. The rate of lateral perineal resection dropped to 8%, ranking among the top in Yuncheng City.
Obstetrical clinic not only undertakes regular projects of common obstetric diseases and frequently-occurring diseases’ diagnosis and treatment, as well as the rescue and treatment of acute and critical diseases; but also carries out risk assessment and prevention of obstetrical deep venous thrombosis, dinoprostone and balloon cervical maturation, multiple suture of postpartum hemorrhage, intrauterine flexible Gyne device placement during cesarean section, uterine balloon compression hemostasis, no lateral incision forceps operation, central venous catheterization monitoring, application of phloroglucinol in obstetrics, etc. In addition, Obstetrics is also responsible for the National Health and Family Planning Commission’s subject “Study on the relationship between vitamin A, E levels and preeclampsia during pregnancy”.
Gynaecology Department
Gynaecology Department has a total of 22 medical personnel, among them there are 2 chief doctors, 3 associate chief doctors, 5 attending doctors, 1 associate chief nurse, 2 chief nurses. The echelon of Departments is reasonable and the technology is in the lead. Under the leadership of Director Zhang Yueqian, we continue to innovate technology and expand our business. On September 14, 2005, the Department completed laparoscopic surgery independently. In 2006, the comprehensive treatment of postpartum hemorrhage with B-lynch was carried out. Rivanol + drug induced labor in the middle of pregnancy was carried out; with high success rate, shortened exclusion time, and reduced postpartum hemorrhage. In 2007, transvaginal minimally invasive surgery was carried out, including total vaginal hysterectomy, transvaginal ovarian tumor resection and transvaginal ectopic pregnancy. Hysteroscopy, laparoscopic diagnosis and treatment of ectopic pregnancy, fallopian tube disease, ovarian tumor surgery and laparoscopic hysterectomy were carried out. Especially after the introduction of transvaginal and laparoscopic vascular ligation device, a variety of minimally invasive surgeries have been widely carried out; so that the pain of the patients is less, the bleeding during the operation is less, the postoperative recovery is fast, and the hospitalization days are obviously shortened. Up to now, minimally invasive surgery has been widely carried out in the Department.
Professor Lang Jinghe from Peking Union Medical College Hospital said that among the three methods of hysterectomy: abdominal surgery, laparoscopic surgery and vaginal surgery, the last one is the least invasive and the most preferred. Hysteromyoma and adenomyosis are very common in gynecologic diseases. With the progress of cervical disease screening program, many cervical precancerous lesions such as cervical intraepithelial neoplasia grade 3 are screened and diagnosed. In addition, there are endometrial precancerous lesions such as atypical hyperplasia of endometrium and uterine prolapse, etc. Many of the above diseases require hysterectomy. Using vagina as a natural lumen for surgery has many advantages, such as less tissue trauma, less interference to abdominal organs, less postoperative pain, less postoperative complications, low medical expenses, etc. It is more in line with the principle of minimally invasive surgery, and its advantages have been recognized. At present, vaginal surgery has become the best choice for hysterectomy. Compared with traditional operation, vaginal hysterectomy has obvious advantages: there is a minimum degree of peritoneum opening, minimal interference to intestinal tract, and low incidence of postoperative intestinal obstruction. There is no wound on the abdomen, leaving no scar on the abdominal wall. The postoperative pain is mild, recovery is quick; patients can move out of bed and eat within 12 hours after operation, and can be discharged within 5 days with less infection, short rehydration time and low medical expenses. Elderly patients, especially those with cardiorespiratory dysfunction and medical complications, have better tolerance to vaginal hysterectomy.
In clinical application of most hospitals, vaginal hysterectomy are limited to patients with uterine prolapse. The main reason is that the operation field is narrow, and indications and contraindications are difficult to master; doctors lack deep operation skills and suitable surgical instruments. In addition, patients lack understanding of the choice of surgical methods. After years of experience in vaginal surgery and introduction of a rapid vascular ligation and closure system, the Department greatly broadens the scope of application of vaginal surgery.
At present, the Department has carried out hysterectomy for more than 10 weeks, transvaginal myomectomy, transvaginal salpingoophorocystectomy and transvaginal ectopic pregnancy. Widening the scope of minimally invasive vaginal surgery has become a technical feature of the Department. Over the past 13 years, vaginal surgery in our department has been recognized by more and more female patients with its unique advantages, making the majority of female patients not only relieve pain but also keep beauty. It is believed that the application of vaginal surgery will be more and more extensive, and we will benefit more female patients with exquisite surgical techniques.
